Holistic Approach to Peri-implantitis Advancing Dental and Maxillofacial Reconstruction

Our research project takes a new, integrated approach to address the challenge of peri-implantitis, focusing on the holistic reconstruction of the alveolar process rather than isolated repair of individual components.

Numerous products are available on the market, not only for dental bone and soft tissue repair, replacement, and regeneration, but also for dental prosthetics and replacement solutions such as dental implants. Most of these products and concepts have existed for well over 50 years. Despite the introduction of rough implant surfaces by Nobel Biocare in 1986, the market has seen few substantial product improvements since then.

Recent advancements have focused on enhancing surface characteristics of metal implants, such as dental screws, by introducing rougher textures, hydrophilic coatings, and improved material compositions. These refinements have contributed to greater implant stability and tissue integration. Nevertheless, peri-implantitis remains a persistent challenge for both clinicians and patients.

A thorough review of scientific literature highlights the importance of patient-related factors, including compliance, diabetes management, and plaque control, in reducing the risk of peri-implantitis. Additionally, innovations in surgical procedures and implant technology—including roughened surfaces and regenerative materials like bone substitutes and collagen membranes—have improved outcomes by reducing implant failure rates. When combined with consistent post-operative care, these strategies promote tissue integration and curb the progression of peri-implantitis. Nevertheless, the increasing rates of peri-implantitis, combined with patient discomfort, implant loss, and rising healthcare costs, demonstrate the urgent need for further improvement.

Implantatentwicklung und die Geweberegeneration werden meist als separate Ziele verfolgt, doch dieser Ansatz hat weder für Patienten noch für klinische Anwender zufriedenstellende Ergebnisse geliefert. Daher bleibt die Periimplantitis ein ungelöstes Problem in der rekonstruktiven Zahnchirurgie. Unser Forschungsprojekt verfolgt einen integrierten Ansatz, indem der Alveolarfortsatz ganzheitlich rekunstruiert wrid, statt eine isolieret Reparatur einzelner Komponenten vorzunehmen.

Our objectives are:

 
• Enhanced Implant Anchorage with Host Tissue: Creating robust, stable connections between implants and host tissue to improve long-term implant performance.
• Regeneration of a Functional Periodontal Ligament or Comparable Tissue: Developing an implant support system that:
- Minimizes bacterial penetration and
- Compensates for mechanical disparities between the implant and natural tissue.
• Accelerated and Sustainable Host Tissue Regeneration: Fostering the development of a mature, load-bearing bone structure that provides a stable anchor for functional periodontal ligament or similar connective tissue.
